Social care teams

As people begin to choose how they would like to be supported, social workers will need to harness each person's expertise - and that of their families and carers - to actively design support solutions that best meet their needs and goals.

As PPF develops, the way social and care workers do their jobs may change, but they will still be focused on helping people achieve the best possible quality of life. Social work practices will continue to prioritise empowerment, encouraging people to shape their own bespoke support. This will enable social and care workers to ensure that individuals receive the right care and support at the right time. PPF will also lead to a better understanding of an individual's whole needs, allowing for earlier interventions before problems arise, and helping people to maintain their health and well being for longer.

Ultimately, you will help to open doors and expand horizons for people needing support.
